In order to assess your suitability for treatments, you will be asked for details of your medical history within your consultation forms. Details of patch tests undertaken and information for any reactions where applicable will be captured.
Should in an emergency medical situation I would share this information with the emergency services in such an event.
This Data is provided by you and held on paper record by me for 10 years following last activity.
